    2.Most happen because of flawed decision-making, bad communications, and poor teamwork or leadership.
    3.Humans trust their own opinion to much, they that they are always right.
    4.when humans get tierd they are starting to make mistakes.
    5.Human performance changes a lot. Human performance does not run smootly.
    6. Beacause they are getting their money and they dont care what is their work outcome. Money is more important.
    7.Some engineres use old techniques to solve modern problems usually these techniques dont work.

    3.For example, impulsivity( for not thinking). I think the most serious deficiency is the lack of character and that when we don´t know what we really want.
    4.For example, the limits of human perception is bounded brain bulk (sense of smell, sight, hearing) and limited brain volume.
    5.For example we get tired and we can´t work as much as robots or machines. We have limited enery supplies.

    3. The top five human personality and thought shortcomings are : anti-authority, impulsiveness, macho attitude, invulnerability and resignation.
    4. Humans make more errors than computers, because humans can be lead to astrays and be easily deceived. Our weakness is also that we are afraid to tell our superior bad news, so we often lie about the truth.
    5. Humans performance shortcoming is that we get fatigued after long-hours of work, especially when we are doing physical and tense work.
    6. We often make poor decisions because we dont want to lose our social or professional position in the group.
    7. Tendency to " solve the wrong problem" means that we look at a problem and we come up with a solution, what actually solves different problem and we apply this solution to our original problem.
    8. "Swiss cheese model" is and organizational model, which is used to analyze the systematic failures and accidents.
    9. I think that 78% of constructional failures refers to human errors and 50% refers to percent of constructional failures which are human caused and involve water.
    10. Best way to minimize engineering errors is not to punish those who have made mistakes. Also engineers should demand higher standards of clarity and usability from publishers of building codes and other complex documents.

    2. Main cause of most aircraft accidents is because of mechanical failures, they are caused by bad decision makeing, bad teamwork, bad communcation or bad leadership. Or all together.
    3. There is five different shortcomings about human personality and thought. a) anti-authority b) impulsiveness c) macho d) invulnerability e) resignation.
    4. Humans errors are the main reasons of failure. Perception could be decrease because drinking too much alcohol or using drugs.
    5. Humans get tired after haveing a long day at work, also we can be affected many different ways, like be distracted by noises or smells ect.
    6. Group could make whoppers beacause similar education, training, beliefs and education.
    7. Engineers must renew their knowledge, because technology gets newer and things change. Knowledge is getting better and because of that engineered buildings and any kind of projects will be stronger and better.
    8. It helps to solve errors, offers opportunities.
    9. 78% - human errors / 50% - water erros. I think that human error numbers can be much smaller what there would be more supervisors.

    5) We take too many risks beacause we want to give our superiors good news
